Redwood Community Health Network (RCHN) is a subsidiary of RCHC. RCHN is a network of RCHC’s Sonoma County members who come together to coordinate in providing high value, high performing collaborative systems of care to improve the health of patients and communities in Sonoma County.
Who our patients are:
Our Sonoma County member health centers provide patient-centered primary care health homes to over 131,500 patients, including over 66,000 Medi-Cal patients and over 33,000 uninsured individuals. Almost 48% of our patients are Latino and roughly 30% of our patients are best served in a language other than English. The vast majority of health center patients are low- and very-low income with 55% of patients having incomes below the federal poverty level (FPL) and only 6% of patients with incomes over the 200% FPL.

New Financing and Reimbursement Models
Establish new value-based reimbursement agreements with managed care plans and other payers

Implement a coordinated health care workforce pipeline and recruitment strategy among RCHN members
Sonoma County Relationships
Invest in the development of strong relationships and alignment with County agencies and major health systems
