Episode 6: PHASE for Care Teams

This episode features Jessicca Moore, director of innovations at Petaluma Health Center who will talk about the PHASE program. PHASE stands for Preventing Heart Attacks and Strokes Every Day which is an evidence-based approach that first helped Kaiser Permanente reduce heart disease morbidity by 60 percent among its members. Aliados Health has participated in the PHASE program since 2011 with funding from the Kaiser Permanente Northern California Community Benefits Program and The Kaiser Foundation Hospital Fund for Community Benefit Programs at the East Bay Community Foundation. In 2020 Aliados Health will participate in an additional round of PHASE, with a new focus on health equity. Aliados Health participating health centers will measure and report on rates of A1c control among uninsured patients with diabetes.
