Vision

Every person in the communities served by Redwood Community Health Coalition health centers has access to a patient–centered health home.

Population Health

RCHC is pursuing triple aim objectives of improving the health populations, improving the experience of care, and reducing per capita cost of health care. The Population Health and Care Innovation Team provides support to member health centers.

Testimonials

“I don’t know what we will do if the Affordable Care Act is repealed and MediCal expansion goes away.”

Miguel and Janet own a small sign and lettering business in Winters and had always had active lifestyles. But when old injuries and health issues (including Miguel’s long-time knee problem) resurfaced, their activity levels began to decrease and they anticipated an increasing need for medical services…

“None of this ‘care’ would have happened without Medi-Cal.”

Kathleen Sarmento is the Director of Nursing at Santa Rosa Community Health. She not only knows what Medi-Cal Expansion has meant to the community-at-large, but also has first-hand, very personal experience: four of her family members have greatly benefited from the coverage.
