Every person in the communities served by Redwood Community Health Coalition health centers has access to a patient–centered health home.
RCHC works with legislators, policy makers, health care providers, health plans, and many other stakeholders to define and shape policies that benefit the health of our communities and the people we serve.
RCHC Certified Enrollment Counselors can answer your questions and help you apply or renew for your health insurance or CalFresh benefits. This process is confidential and provided at no cost. Assistance is available in Marin, Sonoma, and Yolo Counties.
RCHC is pursuing triple aim objectives of improving the health populations, improving the experience of care, and reducing per capita cost of health care. The Population Health and Care Innovation Team provides support to member health centers.
