Quality Improvement (QI)
RCHC’s long-range vision is to have a healthy population with no health disparities in the four counties (Napa, Yolo, Marin, and Sonoma) that we serve. To reach this goal, RCHC and our member clinics need to continue to embrace a culture of excellence through QI to promote quality evidence-based care and actively aid our patients to become involved in their own care. We need to “provide the right care to the right people at the right time” to insure good health outcomes. This vision grows directly from the Redwood Community Health Coalition’s mission and commitment to its member health centers to “increase access to care, improve patient health outcomes, and achieve cost efficiencies, through meeting the needs of our clinic members’ diverse, multi-cultural populations through integration of services, education, and advocacy”.
The QI Department of RCHC serves its members through the management of quality improvement-related grants, providing technical assistance to its member clinics, and participation and leadership in statewide quality improvement initiatives. Current and past clinical grants managed by the department include:
- Department of Health Services Immunization grant
- California Primary Care Association Emergency Preparedness grant
- The California Endowment TIDES grant for the Clinical Systems Learning Community
- California Primary Care Association/California Healthcare Foundation Accelerating Quality Through Quality Improvement grant
- Kaiser Permanente grant for Quality Improvement
Technical assistance directly to member clinics during onsite visits in addition to convening and leading clinic members' peer group forums including:
- Process Improvement Committee
- Specialty Referral Coordinators
- Immunization Coordinators
- Emergency Preparedness Coordinators
