i2iTracks Disease Management Software
Since 2004, RCHC clinics have used i2i Tracks software. This software tool allows our clinicians to quickly assess and manage a large patient population with a variety of chronic conditions. Since the projects inception use of the Tracks system has spread to many additional health centers.
 Use of the system has expanded and additional chronic conditions are now actively managed using the computer based tool. RCHC health centers are continually using systems such as Tracks to expand the integration of quality improvement (process review & redesign) and technology to further advance patient care.
The RCHC TSO manages a central data center that houses the software and clinical databases. In cooperation with the vendor the RCHC staff offer end user support and training for health center staff; helping enhance the quality care our member clinics give the patients.
