Redwood Community Health Coalition (RCHC) is pursuing triple aim objectives of improving the health populations, improving the experience of care, and reducing per capita cost of health care.

The Population Health and Care Innovation (PHCI) Team provides support to member health centers in the areas of population health, data analytics and quality improvement through various services, programs and initiatives. We also support care innovation and collaboration among health centers and our health care partners in order to transform and improve quality of care and health outcomes for the individuals and communities we serve. RCHC services include:

  • Best Practice Sharing and Member Collaboration
  • Collaboration with Health Systems Partners for Care Coordination

 

RCHC member health care organizations and other qualified parties participate in an Organized Health Care Arrangement (OHCA) as defined by the Health Insurance Portability and Accountability Act (HIPAA)for health information exchange. An OHCA allows participants to share protected health information to treat their patients, coordinate patient care between different health care settings, and to manage their joint operations.
